I agree to have eyelash extensions, lifting and/or tinting applied to my natural eyelashes and/or removed and retouched. By signing this agreement, I consent to the placement and/or removal of the eyelash extensions by the technician.
I will take in to account recommendations in regards to length, curl and design made by my technician in their professional opinion. Shall I wish to proceed against their recommendation I agree that I hold all accountability for any/all issues that could occur from this which may include but is not limited to: Irritation, damage to natural lashes and/or undesirable results.
I understand and agree to the after-care instructions provided by the technician for the use and care of my eyelash extensions. I understand and accept that not following these instructions may cause the eyelash extensions to fall out and/or decrease the retention time as well as have the potential to result in infections. I understand that rectifying any issue caused by my own mistake or negligence will be at my own cost.
CONTRAINDICATIONS, REACTIONS AND PATCH TESTING: All products used are of the highest quality and are safe to use in accordance with Australian Standards. Whilst very rare it is possible, even with the most experienced technician, to experience an allergic reaction to eyelash extensions, lash lifting and lash/brow tinting products. If you would like you can request a patch test and we will perform this 24hrs before your appointment. It is important to remember that a patch test does not guarantee a sensitivity free service as there have been many instances where a person has had no reaction to their patch test but then had a reaction to their full service. It is also possible to become intolerant to the products even after years of receiving this treatment.
Allergic reactions are outside of our control and we don't offer a refund if this happens to you. It is important to understand that you are proceeding at your own risk, so please ensure to do your research and ask questions to ensure this is the right treatment for you.
I understand that there are risks associated with having artificial eyelashes and eyelash extensions applied to or removed from my natural eyelashes. I further understand that in rare cases as part of the procedure eye irritation and discomfort could occur. I agree that if I experience any of these conditions with my lashes that I will contact the technician that performed this procedure and it may be beneficial to have the eyelashes removed.
This agreement will remain in effect for this procedure and all future follow-ups conducted by the technician. I understand that this consent agreement is legally binding. I have read and fully understand all information in this agreement and hereby release any and all persons representing this salon from all claims, demands, damages, actions and cause of action arising out of the performance of the service.
I have fully disclosed all conditions regarding my health history, medications and past reactions to products, treatments and medications.
I am over 18 years of age and consent to the agreement and to the eyelash extension/lash lifting/lash and brow tinting/hair threading application procedure.
In addition, I agree to all terms, conditions and salon policies available on our website.
